Do you Qualify for a Grant?

Must be actively employed or on an approved leave of absence at the time of the qualifying event.

Applications should be submitted online within sixty (60) days of the qualifying event.

May be eligible to receive a grant once within any six (6) month period, up to $2,500 with a lifetime maximum of $5000.

Program does not provide financial assistance for homelessness, eviction, loss of wages, or food insecurities.

Assistance is not provided in advance of a surgery or medical procedure.

May not be eligible for a grant, if receiving income or insurance reimbursement (e.g., workers compensation, insurance homeowners/renters, life insurance etc.…)

Program does not cover medical premiums, co-payment, deductible, or bills.

Employee's or Employee’s Spouse’s name must be listed on the bill as a responsible party.

If approved for a grant, must be employed when the grant funds are disbursed. Funds are disbursed through PayPal.

Critical Health Condition – When illness or injury prevents you from working for a period of two weeks or more.

Death of employee’s immediate family member or employee – Funeral or cremation bill assistance

Natural Disaster – When your home and/or essential items are destroyed by a natural disaster (hurricane, tornado, fire, etc.)

Intimate Partner Violence – Assistance to vacate and secure a new residence due to physical abuse by an intimate partner.
